Get Certified From a Physician

To obtain your medical card you will need to receive a Physician Certification from a Physician or Medical Marijuana Facility. Your local trusted Physician should be able to help you with this. We also recommend our own trusted Physicians. See the list below to connect with one and set up an appointment for certification today.

Apply For Your Card

Once you’ve obtained a certification from your physician you will need to submit your application online with all the required document the DHSS requires. They aren’t accepting mailed applications currently.

If you are a parent or guardian applying for a patient you will also need this Consent Form.

Note: If the qualifying patient is a minor the parent or legal guardian will need to provide their name, SSN, Parent/Legal Guardian Consent Form and will act as the Primary Care Giver for the patient.
